All adults who volunteer with minors need to go through a background screening. This process includes a background check, interview and reference check.
The background screening forms are available for download below or can be picked up from the church office. Please note that all three forms must be completed in order to process the background screening.
Please compete all three forms and return them in a sealed envelope to Carrie Fugett, Children's Ministry Assistant. All information is kept confidential.
