All adults who would like to volunteer in Children's Ministry are required to complete our background check forms.  Please note that all three of the below forms must be completed in order to process the checks.

You may download these forms and return them to the church office to the attention of Carrie Fugett, Children's Ministry Assistant.  They are also available in the church office.  All information is kept confidential.
